EXTENSION REQUESTS

COMMONWEALTH SPONSORED PLACE + PRAC PAYMENTS

The Australian Government subsidises part of the cost of your tuition fees through limited available Commonwealth Supported Places (CSP). CSP availability depends on the course and your eligibility. Eligible students pay the remaining amount, which may be deferred through a HECS-HELP loan if eligibility requirements are met.

The Commonwealth Prac Payment (CPP) provides financial support to eligible students undertaking mandatory practical placements as part of their studies. The payment is designed to help ease the financial pressure of completing a placement, particularly when balancing study with work and other commitments. This form can be used by MPH, DMN and PhD Candidates to manage their enrolment and payments.

COMPLAINTS AND GRIEVANCE FORM

Please fill out the Complaints and Grievance form to lodge an official complaint or grievance to AC.

 

STUDENT CARD FORM

Please fill out the Student Card form to receive your official AC student card.

 

APPLICATION FOR STUDENT TRAVEL CONCESSIONS

Please complete the Student Travel Concession Card Application Form if you believe you are entitled to a Student Travel Concession. This form is for students in Sydney, Perth, and Adelaide campuses only. If you are a student at one of our other campuses, please see the Student Services staff at your local campus to arrange your travel concession.

 

GRADUATION FORM

Please complete this Graduation Application Form to indicate how you would like to receive your Graduation documentation. Once submitted, your eligibility will be reviewed. You will receive a confirmation email advising whether you will be included in the upcoming Graduation or if you have further classes to complete to become eligible.
